    Documentation Required: Photo I.D. for adults, for those 18 years or younger one form of the following: birth certificate, medical card, etc. Verification of address (i.e. utility bill, lease agreement, or piece of mail). Proof of income, ex: a pay stub, the award letter for SSI or SSDI, or a letter from a boss stating salary.

    Hours:
    Monday - Thursday
    8:30am - 11:00am; 1:00pm - 3:00pm
    The 3rd Saturday of each month
    9:00am - 11:00am only
