    Each adult in the household must provide a photo ID with their DOB as well as piece of mail received at current address within the last 30 days. For minors you must provide a birth certificate, school record, or medical record with the parent's name and current address.

    Monday and Wednesday only from 2:00 pm to 5:00pm
